Enter your TIN (SSN or EIN) correctly — it must match IRS records and the name in Box 1.

Check the correct tax classification in Box 3 (e.g., S-Corp, C-Corp, or Partnership).

A disregarded entity should instead check the appropriate
box for the tax classification of its owner.​​

Minimum limits: $1,000,000 per occurrence / $2,000,000 aggregate -
Workers' Compensation and Employers' Liability
If you are a sole proprietor, partnership only consisting of owners, or an individual with no employees, you will need to apply for a Workers' Compensation Coverage Waiver for the state the work will be performed in. No work may be done without a current Workers' Compensation coverage or waiver on file with LTS.-
Eligibility varies by state and business structure. It is your responsibility to obtain and submit valid proof (e.g., waiver certificate, exemption letter, or active policy). If the state the work will be performed in is not listed above, please contact that state's Workers' Compensation department.
